HIV preexposure prophylaxis among adolescents in the US

Yusuf H, Fields E, Arrington-Sanders R, et al. - Via performing this review, researchers examined the current status of preexposure prophylaxis (PrEP) uptake among at-risk adolescents aged 13 to 19 years as well as assessed recommendations for enhancing PrEP access, uptake, and future required directions, including specific recommendations for health care professionals. In 2018, a total of 37,377 new HIV diagnoses were made; 7,734 of these diagnoses (21%) occurred in adolescents and young adults aged 12 to 24 years; these comprised 1,707 diagnoses (22%) made in adolescents aged 13 to 19 years. Adolescents with the greatest risk for HIV had lowest preexposure prophylaxis awareness and engagement. HIV testing is not routinely offered as recommended by the Centers for Disease Control and Prevention and sexual risk exposures of patients are not routinely assessed by the adolescent primary care clinicians and specialists. Clinicians’ perceptions of the patient’s HIV risk and their knowledge and acceptance of PrEP guidelines often guide their decision to prescribe PrEP for adolescents. Across jurisdictions, access and uptake are often limited because of differences in state laws on consent, confidentiality, and the rights of the adolescent to independently access PrEP outside of parental influence. As PrEP use is important for HIV prevention in adolescents at risk for HIV, they emphasize optimization of its uptake which includes enhancing clinicians’ knowledge about HIV risk and prevention strategies, improving sexual history taking and risk assessment through training and retraining, and enhancing PrEP knowledge and acceptance of prescribing among clinicians. In adolescents at greatest risk for HIV, HIV transmission can be reduced by leveraging the ubiquity of social media, encouraging family support, and performing research aimed at finding lifestyle-congruent formulations.
