Histomorphology of pancreatic cancer in patients with inherited ATM serine/threonine kinase pathogenic variants

Hutchings D, et al. - The experts studied the histomorphology of pancreatic cancer in 23 subjects with germline ATM (a gene correlated with an increased risk of pancreatic ductal adenocarcinoma) pathogenic variants. The histologic subtype was ductal adenocarcinoma, adenosquamous carcinoma, and colloid (mucinous non-cystic) carcinoma in 19/23 (83%), 1/23 (4%) and 3/23 (13%) of the subjects. In subjects with familial and sporadic pancreatic cancer, the percentage of colloid (mucinous non-cystic) carcinomas was greater than formerly observed. Three carcinomas (2 colloid carcinomas, 1 ductal adenocarcinoma) arose in correlation with intraductal papillary mucinous neoplasms. Non-invasive precursor lesions, including pancreatic intraepithelial neoplasia and incipient intraductal papillary mucinous neoplasms, were recognized in 83% among the resected pancreata. Hence, among subjects with germline ATM pathogenic variants, pancreatic cancers were more commonly of colloid (mucinous non-cystic) morphology, however, they were overall morphologically diverse promoting the advantage of universal germline genetic testing for subjects with pancreatic cancer.
