Higher LRINEC scores and escalation of anesthesia care in necrotizing soft tissue infection

Tessler RA, et al. - Via a retrospective cohort study of first operative debridement for suspected necrotizing soft tissue infection (NSTI) in a single referral, researchers examined if higher preoperative Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) scores are associated with escalations of intraoperative anesthesia care in NSTI. Further, they determined the possible use of this score in communicating illness severity during patient handoffs. Three hundred thirty two patients undergoing their first operative debridement for suspected NSTI were identified. A higher risk of norepinephrine and vasopressin use, packed red blood cell use, and additional crystalloid was observed for every 1-point higher LRINEC score. A requirement for vasopressors or blood was unlikely among those with a low score. They identified this score as useful in standardized handoff tools for patients with NSTI.
