High vs low dose caffeine in preterm infants: A systematic review and meta-analysis

Brattström P, et al. - Researchers assessed high vs low dose of caffeine via a systematic review and meta-analysis. MEDLINE, EMBASE, Central, and conference proceedings were examined for randomized controlled trials (RCTs) and quasi-RCTs. They observed that the use of high dose did not influence mortality or bronchopulmonary dysplasia, though it seems to result in fewer cases of extubation failure and apneas and shorter period of mechanical ventilation. Ultimately, the quality of the evidence was low due to imprecise estimates, so researchers could not properly assess if high dose caffeine is more powerful and safe than a low dose.
