High plasma renin activity associates with obesity-related diabetes and arterial hypertension, and predicts persistent hypertension after bariatric surgery

La Sala L, Tagliabue E, Vieira E, et al. - Researchers assessed the renin–angiotensin–aldosterone system (RAAS) in correlation to anthropometric and metabolic variables, with special reference to glucose tolerance, among severely obese individuals. Assessment of 239 people was done at baseline and of 181 one year post-bariatric surgery [laparoscopic gastric banding (LAGB)]. Post-LAGB, reduction of PRA (plasma renin activity) as well as aldosterone was significant in hypertensive, but not in normotensive participants, and was shown to correlate with reduction of ferritin. Persistent hypertension 1 year post-LAGB was predicted by PRA and glucose levels. The findings lend support to RAAS role in pathophysiology of glucose homeostasis, and in the regulation of blood pressure in obesity. There could be a contribution of ferritin (as a proxy of subclinical inflammation) to the cross-talk between RAAS and glucose metabolism.
