High cryptococcal antigen titers in blood are predictive of subclinical cryptococcal meningitis among HIV-infected patients

Wake RM, et al. - In this study, researchers explored whether high mortality rates among asymptomatic cryptococcal antigen (CrAg)-positive patients identified through CrAg screening, despite pre-emptive fluconazole, were due to undiagnosed cryptococcal meningitis. There were around a third of asymptomatic CrAg-positive patients who had concurrent cryptococcal meningitis. They realized the necessity for more effective clinical assessment strategies and antifungal regimens for CrAg-positive patients, including investigation for cryptococcal meningitis irrespective of symptoms. Where not possible to perform LPs on all CrAg-positive patients, they recommended using blood CrAg titers to target those most at risk of cryptococcal meningitis.
