High cervical spinal cord stimulation for occipital neuralgia: A case series and literature review

Texakalidis P, et al. - Researchers sought to report on the effect of high cervical spinal cord stimulation (SCS) as a treatment modality for refractory occipital neuralgia (ON) via reviewing their own experience and conducting a systematic literature review of studies addressing this topic. They treated five patients with refractory ON with high cervical (C1–C3) SCS and achieved a successful trial stimulation (> 50% pain reduction) in two out of these five (40%) patients who then received permanent implantation. These patients displayed a decrease in the visual analog scale score from 7.5 to 4 and from 6.5 to 5 with no reported complications for any of the patients. For the systematic literature review, they identified two suitable studies, comprising 18 patients overall on whom cervicomedullary junction SCS was performed. A successful trial stimulation (> 50% pain reduction) was reported in nine out of 18 patients (50%) who then received permanent implantation. Outcomes thereby support the association of high cervical or cervicomedullary junction SCS with a 40%–50% successful trial rate in refractory ON.
