Hemodialysis without anticoagulation: Less clotting in conventional hemodialysis than in predilution hemodiafiltration

Krummel T, et al. - Researchers intended to confirm the efficacy of predilution hemodiafiltration (HDF) as an alternative to heparin in hemodialysis patients with contraindication to systemic anticoagulation. Random assignment of participants ie, patients hospitalized in a nephrology ICU and hemodialysed for stage 5 CKD or AKI and with contraindication to systemic anticoagulation, to either standard HD with a polysulfone membrane, or to predilution HDF with the same membrane was done. Findings revealed more premature clotting occurring in relation to predilution HDF vs conventional HD without improvement in dialysis duration or performance epuration indices. Also, the proposal of conventional heparin-free hemodialysis was found safe in most patients with high bleeding risk, when aiming for a 4-hour duration session.
