HCV core antigen as an alternative to HCV RNA testing in the era of direct-acting antivirals: Retrospective screening and diagnostic cohort studies

The Lancet: Gastroenterology & Hepatology Dec 12, 2018

van Tilborg M, et al. - In this retrospective screening cohort study, the investigators evaluated the efficiency and cost assumptions of using chronic hepatitis C (HCV) core antigen testing to substitute HCV RNA testing for validation of diagnosis, on-treatment monitoring, and measurement of sustained virological response (SVR) from Aug 1, 2014, to Feb 28, 2015. De-identified residual serum from unselected samples obtained from commercial laboratories in Ontario, Canada, were tested and it was concluded that HCV core antigen testing could be used instead of HCV RNA testing for diagnosis and documentation of treatment adherence, but it might not be enough to determine SVR. This approach was thought useful in gaining access to care, especially in low-income and middle-income nations.
