Haemodynamic stability after paracervical block: A randomized, controlled, double-blind study comparing bupivacaine-adrenaline with bupivacaine

Sjoeen GH, et al. - In this randomised, double-blinded, controlled study, researchers assessed haemodynamic stability in 30 healthy women who were scheduled for cervical conisation and received a paracervical block using bupivacaine with and without adrenaline. After induction of general anaesthesia, two groups were defined: the BA-group including 14 women who received bupivacaine 50 mg with adrenaline 100 μg; and the B-group including 16 women who received bupivacaine 50 mg without adrenaline. Minimally invasive haemodynamic monitoring was performed using LiDCOplus. The main outcomes of interest were changes in cardiac output (CO) and systolic blood pressure (SBP) as well as changes in heart rate (HR), stroke volume (SV), and systemic vascular resistance (SVR). Findings revealed that haemodynamic instability could be induced by paracervical block with bupivacaine 50 mg and adrenaline 100 μg in healthy females, and therefore, this strategy should not be used if haemodynamic side effects are to be avoided.
