Haemodynamic mechanisms and long-term prognostic impact of pulmonary hypertension in patients with severe aortic stenosis undergoing valve replacement

Weber L, et al. - Researchers assessed the detailed hemodynamics, prevalence, and prognostic influence of pulmonary hypertension (PH) in severe aortic stenosis (AS) via studying 503 patients (mean age 74 ± 10 years) with severe AS (indexed aortic valve area 0.4 ± 0.1 cm2/m2, left ventricular ejection fraction 57 ± 12%) having left and right heart catheterization before aortic valve replacement. Findings revealed a common occurrence of PH in patients with severe AS, but with different underlying mechanisms. Patients with combined pre- and post-capillary PH (CpcPH) have higher mean pulmonary artery wedge pressure (mPAWP), lower pulmonary arterial capacitance, and worse survival vs those with isolated post-capillary PH, pre-capillary PH, and without PH. High mPAWP in CpcPH seemed to be driven by left atrial dysfunction and mitral regurgitation.
