Graft choice in isolated medial patellofemoral ligament reconstruction: A systematic review with meta-analysis of rates of recurrent instability and patient-reported outcomes for autograft, allograft, and synthetic options

McNeilan RJ, et al. - Rates of recurrent instability as well as postoperative complications (other than instability), and subjective symptom improvement after isolated medial patellofemoral ligament (MPFL) reconstruction were analyzed in relation to graft selection or patient age, via meta-analysis. Findings demonstrated that autograft was not superior to allograft or synthetic grafts, and rates of recurrent instability were generally low. Data showed that regardless of graft selection, isolated MPFL reconstruction has the ability to afford significant symptom relief, although there is a bias toward reporting better than expected results among smaller studies. Higher recurrent instability rates were reported among pediatric patients and patients treated with adductor tendon autograft. Notably, it is appropriate to base the selection of graft type on surgeon preference, comfort, and prior experience.
