Glaucomatous progression after lens extraction in primary angle closure disease spectrum

Song MK, Sung KR, Shin JW, et al. - Researchers conducted this retrospective observational study to examine the longitudinal clinical course of the primary angle closure (PAC) disease spectrum, including PAC, primary angle closure suspect (PACS), and primary angle closure glaucoma (PACG) patients, who had lens extraction. They involved 77 eyes from patients with PACS, PAC, and PACG who had lens extraction and were followed up for > 2 years after surgery. Seventeen PACS, 24 PAC, and 36 PACG eyes have been involved. In all groups, the mean and peak IOP, and IOP fluctuation IOP were significantly reduced postoperatively. Lens extraction achieved significant IOP reduction in the PAC spectrum. A substantial proportion of PACG eyes, however, displayed glaucomatous progression despite IOP control. This finding may indicate that lens extraction should be performed early in order to avoid PACG.
