Gland-preserving surgery for salivary stones and the utility of sialendoscopes

Fabie JE, et al. - Researchers assessed the predictors of failure in the endoscopic management of sialoliths among 206 subjects. They categorized the subjects treated for sialolithiasis into 3 groups: successful interventional sialendoscopy, incisional sialolithotomy, and gland excision. They attempted interventional sialendoscopy in 156 cases whereas it was successful for 42 candidates. As compared to incisional sialolithotomy (7.90 mm), smaller calculi were retrieved endoscopically (7.90 mm vs 4.96 mm). A frequent requirement of nonendoscopic approaches was noted in 87% of submandibular cases. They detected palpable stones in 74% of incisional sialolithotomies. They found the following factors predictive of incisional management: submandibular location, palpability, CT localization, and increased diameter.
