Genomic characterization of breast fibroepithelial lesions in an international cohort

Md Nasir ND, Ng CCY, Rajasegaran V, et al. - A total of 303 fibroadenomas (FAs) and 493 phyllodes tumors (PTs) which were contributed by the International Fibroepithelial Consortium and 16-gene panel that was earlier found to be implicated in pathogenesis and progression were included in this study by the researchers in order to characterize a large international cohort of Fibroepithelial lesions (FELs) through targeted sequencing. Genetic aberrations were significantly related to rising grade of PTs, and were discovered more in PTs vs FAs for MED12, TERT promoter, RARA, FLNA, SETD2, TP53, RB1, EGFR, and IGF1R. Most borderline and malignant PTs had ≥ 2 mutations, while there were more cases of FAs with ≤1 mutation in comparison with PTs. FELs with MED12 mutations had significantly greater rates of TERT promoter,RARA, FLNA, SETD2, TP53, RB1, EGFR, and IGF1R aberrations. Nonetheless, FELs with wild-type MED12 were more inclined to express TP53 and PIK3CA mutations. No important variations was noted between the mutational profiles of recurrent FAs, FAs with history of subsequent ipsilateral recurrence or contralateral occurrence, and FAs without a history of subsequent events. Recurrent mutations which were more prevalent in PTs vs FAs, with borderline and malignant PTs harboring cancer driver gene and multiple mutations were recognized. Hence, the role of a set of genes in FELs, including its potential use in classification on the basis of mutational profiles was declared.
