Genital tuberculosis screening at an academic fertility center in the United States

Tal R, Lawal T, Granger E, et al. - Via this prospective cohort study, researchers sought to report on the prevalence of female genital tuberculosis in infertile patients at their academic fertility center. Between January 2014 and January 2017, 323 infertility patients (7.7%) were screened for latent tuberculosis; of these, 25 (7.7%) had positive QuantiFERON-TB Gold results. This suggests a low prevalence of female genital tuberculosis in infertile women in the United States. However, without utilization of multiple diagnostic modalities including endometrial sampling, it can be underdiagnosed. In view of the potential for serious maternal and neonatal morbidity in affected patients using assisted reproductive technology, they emphasize screening for latent tuberculosis of all at-risk women seeking infertility care in the United States. In patients who screen positive, they recommend performing endometrial biopsy for evaluation by histology, polymerase chain reaction, and culture for Mycobacterium tuberculosis to rule out female genital tuberculosis before initiation of infertility treatments.
