Furosemide for accelerated recovery of blood pressure postpartum in women with a hypertensive disorder of pregnancy

Perdigao JL, Lewey J, Hirshberg A, et al. - In view of a significant causative role of persistent postpartum hypertension in maternal morbidity, researchers here examined how furosemide affects postpartum blood pressure recovery in women with hypertensive disorders of pregnancy. In this randomized, double-blind, placebo-controlled trial, women with gestational hypertension and preeclampsia with/without severe features received a 5-day course of 20 mg oral furosemide or placebo. Randomization of 384 women was done (192 per group). In women randomized to furosemide, a 60% reduction in the prevalence of persistently raised blood pressure was observed at 7 days when controlling for cesarean. Overall findings from this double-blinded randomized trial revealed significantly improved blood pressure control in women with hypertensive disorders of pregnancy, mostly among women without severe disease, in correlation with receiving a short course of postpartum furosemide.
