FRIEND or FOE: A prospective evaluation of risk factors for reintubation in surgical and trauma patients

Michetti CP, et al. - Using a Form for Re-Intubation Evaluation by Nurses and Doctors (FRIEND), researchers prospectively collected pre-extubation data, to determine failure of extubation (FOE) risk. From 1/1/16 to 5/31/17, completion of FRIENDs, including airway, breathing, and neurologic variables, was performed before extubation on trauma & surgical patients in one ICU. Comparison of those with failed vs successful extubation revealed that patients with FOE had higher ICU days, ventilator days, and mortality. Increased odds of FOE were observed with age, delirium, moderate/copious secretions, and enteral opioid use.
