Free perforator flaps vs propeller flaps in lower limb reconstruction: A cost/effectiveness analysis on a series of 179 cases

Innocenti M, et al. - Researchers performed a comparison of free perforator flaps vs propeller flaps in the coverage of lower limb soft tissue defects. They evaluated 179 patients (age between 5 and 92 years old) who underwent soft tissue reconstruction of the lower limb from January 2009 to January 2015, either by free flap or propeller flap. As per outcomes, the two groups were not significantly different about the correlation of risk factors or flap size with complication or failure. They recommend undertaking the surgical option choice only after an accurate evaluation of the soft tissue surrounding the defect. Preference should be given to propeller flaps in case of small/medium size defects in otherwise healthy extremities. In large defects due to trauma or vascular diseases, they support using free perforator flaps. Consideration to propeller flap should be given whenever possible in view of the economic analysis outcome.
