`Frailty as a predictor of outcomes in patients undergoing carotid artery stenting

Faateh M, Kuo PL, Dakour-Aridi H, et al. - Researchers aim at examining the correlation between the 5-item modified frailty index (5mFI), functional status, and peri-operative outcomes among patients who underwent carotid artery stenting (CAS) in VQI between November 15, 2016 to December 31, 2018. They defined good function status as being able to carry on all pre-disease activities without restriction using a new variable added to VQI. Calculation of 5mFI was done utilizing functional status and histories of diabetes, COPD, CHF, and hypertension. Among the 7,836 assessed patients, 188 patients (2.4%) had peri operative stroke/death, 765 patients (9.8%) had non-home discharge, and 2,584 (33.0%) had prolonged postoperative stay. Findings suggest independent predictive value of frailty as measured by the m-5FI and functional status for perioperative stroke/death, non-home discharge and increased length of stay in patients undergoing CAS. In asymptomatic patients, these results were noted to be greatly pronounced. Hence, they emphasize practicing caution against the use of CAS in asymptomatic frail patients.
