Focal osteonecrosis in the femoral head following stable anatomic fixation of displaced femoral neck fractures

Lazaro LE, et al. - In a study population sustaining displaced femoral neck fractures (FNF), researchers quantified residual perfusion to the femoral head (FH) and, assessed the viability of the FH overtime after surgical fixation. Findings reported high incidence of small osteonecrosis (ON) segments in the supero-medial quadrant of the FH, resulting from significantly impaired vascular supply to the FH due to FNF. Additionally, despite a high rate of small ON segments noted on MRI, excellent clinical and radiographic outcomes were visible as a result of maintained perfusion, probably via the inferior retinacular system, coupled with urgent open anatomic reduction and stable fixation.
