Five-year outcome of operative and non-operative management of meniscal tear in persons greater than 45 years old

Katz JN, Shrestha S, Losina E, et al. - 5-year follow-up data from the MeTeOR (Meniscal Tear in Osteoarthritis Research) Trial of physical therapy vs arthroscopic partial meniscectomy (APM) was analyzed by the researchers in order to explore five-year outcomes of treatment of meniscal tear in OA. A total of 351 participants were randomized. Knee Osteoarthritis and Injury Outcome Score (KOOS) Pain scores were ~46 at baseline in both arms, in the intention-to-treat (ITT) analysis. In both arms, over the first three months, pain scores got better substantially, and persisted to improve through 24 months (to ~18 in each arm) and were stable from 24 to 60 months. Results of the as-treated analyses of KOOS Pain were comparable. Over five years, 25 participants underwent total knee replacement (TKR). Therefore, in both arms, over 60 months, the pain got better considerably. While ITT analysis exhibited no statistically important variations in TKR use, higher TKR use in those receiving APM warrants additional study.
