Five year kidney outcomes of bariatric surgery differ in severely obese adolescents and adults with and without type 2 diabetes

Bjornstad P, Nehus E, Jenkins T, et al. - Given that improved markers of kidney health in relation to bariatric surgery have been seen in severely obese individuals, still, it remains unclear if kidney disease results vary according to age at surgery, so, researchers performed this study with 161 adolescents and 396 adults engaging in two related but different studies, to determine health effects of Roux-en-Y gastric bypass in these individuals. High urine albumin-to-creatinine ratio (UACR) of 30mg/g or more and hyperfiltration (an estimated glomerular filtration rate of 135 ml/min/1.73m2 or more) were the main outcomes. A significantly raised prevalence of high UACR before surgery was noted in adolescents with preoperative type 2 diabetes vs adults. A significantly increased prevalence of hyperfiltration, which continued throughout the study duration, was seen in adolescents with preoperative type 2 diabetes, whereas, similar prevalence of hyperfiltration was reported among those without type 2 diabetes. Overall, earlier attenuation of elevated UACR was observed in adolescents with preoperative type 2 diabetes vs adults with preoperative type 2 diabetes in response to gastric bypass.
