Fibrinolysis vs primary percutaneous coronary intervention for ST‐segment elevation myocardial infarction cardiogenic shock

Vallabhajosyula S, Verghese D, Bell MR, et al. - Researchers utilized the National (Nationwide) Inpatient Sample from 2009 to 2017 in this large national observational study to compare outcomes among adult (> 18 years) ST‐segment elevation myocardial infarction cardiogenic shock (STEMI‐CS) admissions receiving initial fibrinolysis vs primary percutaneous coronary intervention (PPCI). Higher rates of haemorrhagic complications were reported in the fibrinolysis group. Comparable rates of acute organ failure, hospital length of stay, rates of palliative care referrals, do‐not‐resuscitate status use, and lesser hospitalization expenditures were found in the fibrinolysis group. In the contemporary era, in‐hospital mortality associated with the use of initial fibrinolysis was comparable to that noted in patients who received PPCI in STEMI‐CS.
