Fibrin monomers and association with significant hemorrhage or mortality in severely injured trauma patients.

Esnault P, Mathais Q, Gueguen S, et al. - As fibrin monomers (FMs) are a direct marker of thrombin action, they reflect coagulation activation. Researchers here investigated how FMs levels at admission are associated with significant hemorrhage and 28-day mortality after a severe trauma. In this retrospective, observational study, they included 299 severe trauma patients admitted in a level-1 trauma center between January 2012 and December 2017. Significant hemorrhage occurred in a total of 47 (16%) patients. Findings suggest poor accuracy of FMs in predicting the occurrence of significant hemorrhage. However, they suggest excellent sensitivity and NPV of FMs that could aid in identifying patients with a low risk of bleeding during hospital care. Further, they indicate a possible association of FM levels ≥ 109.5 µg/ml with the 28-day mortality.
