Fetal head station at second-stage dystocia and subsequent trial of labor after cesarean delivery success rate

Rottenstreich A, Meyer R, Rottenstreich M, et al. - Researchers conducted a retrospective cohort study of 481 primiparous women with subsequent delivery after cesarean delivery for second-stage dystocia in order to determine if fetal head station at the index cesarean delivery is linked with a subsequent trial of labor success rate among primiparous women. Among these women, 64.4% (n = 310) attempted trial of labor after cesarean, and 222 (71.6%) successfully delivered vaginally. Lower fetal head station at the index cesarean delivery for second-stage dystocia was noted to be independently linked with a higher vaginal birth after cesarean rate, with an overall acceptable success rate. These findings may facilitate improving patient counseling and reassuring those who wish to deliver vaginally after prior second-stage arrest.
