Fertility-sparing surgery for treatment of non-epithelial ovarian cancer: Oncological and reproductive outcomes in a prospective nationwide population-based cohort study

Rodriguez-Wallberg KA, et al. - Researchers used the Swedish Quality Register for Gynecological Cancer to select all women (ages 18–40) who underwent fertility-sparing surgery (FSS) or radical surgery (RS) for treatment of non-epithelial ovarian cancer (NEOC) between 2008 and 2015, to compare the oncologic outcome between FSS vs RS in this prospective, nationwide, population-based study. They also describe the reproductive outcomes in women following FSS. A diagnosis of stage I NEOC was received by 73 women and the majority, 78% (n = 57), underwent FSS. Following RS, the recurrences were more common as compared with FSS. In young women with early stage NEOC, no link of FSS with worse oncologic outcomes, compared with RS, was identified in this study. Both groups displayed excellent prognosis, with an overall survival of 98%. In women who underwent FSS, maintained natural fertility was reported, only 12% needed assisted reproductive technology treatment.
