Femoral nerve blocks for the treatment of acute prehospital pain: A systematic review with meta‐analysis

Raatiniemi L, Magnusson V, Hyldmo PK, et al. - By performing this systematic review, researchers compared femoral nerve block (FNB) with other forms of analgesia (or no treatment), in terms of effect and safety, for managing acute lower extremity pain in adult patients in the prehospital setting. The outcomes were evaluated employing the Cochrane and GRADE methods. This analysis involved 4 studies with 252 patients. As per 2 RCTs (114 patients), a more effective pain reduction may be achieved with FNB vs metamizole. FNB with lidocaine and magnesium sulphate was compared with FNB with lidocaine alone in one RCT (48 patients), and it was only incorporated here for information about adverse effects. One case series involved 90 patients. The analyzed studies revealed few adverse events. This work shows that there exists a limited evidence concerning the effectiveness as well as adverse effects of prehospital FNB. Experts also noted a lack of investigations comparing prehospital FNB vs inhaled analgesics, opioids or ketamine.
