Feasibility and accuracy of speckle tracking echocardiography in Emergency Department patients: A preliminary study

Reardon L, et al. - Speckle tracking echocardiography (STE) delivers an objective measurement of myocardial strain better than visual assessment of wall motion when done as part of a point-of-care (POC) echocardiogram, so researchers investigated if POC STE operated by emergency providers is feasible and accurate when compared to comprehensive echocardiography for final diagnosis of ACS. They performed a retrospective review of 187 emergency provider POC echocardiograms with STE from July 2014–May 2016 for suspected ACS at a large academic trauma center. In emergency department (ED) patients with suspected ACS the potential feasibility of STE in addition to standard bedside echocardiography when operated by experienced ultrasound-trained physicians in the ED. For diagnosing ACS, STE done by emergency providers is not sensitive enough, and has low accuracy vs comprehensive echocardiography. However, when performed by expert ultrasound-trained providers, improvement in the positive predictive value and specificity was observed. They recommend considering STE for inclusion in the Emergency Ultrasound Fellowship curriculum.
