FDG PET/CT evaluation of pediatric patients with yolk sac tumor

Tang W, et al. - Data from 31 pediatric patients with pathologically validated yolk sac tumor (YST) who underwent 34 PET/CT studies was recruited by the researchers in order to assess the clinical utility of FDG PET/CT in staging and restaging pediatric patients with YST. In all six staging studies, FDG PET/CT was true-positive, detected only a few more metastatic foci than conventional imaging, and altered the therapeutic regimen in none of the six patients. Nonetheless, PET/CT exhibited high exactitude in the restaging group, with a sensitivity and specificity of 100% and 85.7%, respectively. In 46.4% of the patients in the restaging group, the treatment regimen was altered according to the PET/CT study. Moreover, in YST restaging, PET/CT had greater accuracy than α-fetoprotein levels. In all, the per-study performance of PET/CT was a sensitivity and specificity of 100% and 85.7% along with the positive and negative predictive value of 90.9% and 100%, respectively. Therefore, in staging YST in pediatric patients, FDG PET/CT was only slightly better to conventional imaging. Nevertheless, the PET/CT of posttherapy patients with YST exhibited high diagnostic accuracy and had a high influence on therapeutic management.
