Favorable renal outcomes after intravenous thrombolytic therapy for acute ischemic stroke: Clinical implication of kidney-brain axis

Chang TH, et al. - Among patients who were hospitalized for acute ischemic stroke from January 2001 to January 2017, researchers analyzed the impact of systemic recombinant tissue plasminogen activator (rt-PA) administration on long-term renal outcomes. Various confounding variables were removed by applying 1:2 propensity score matching. They defined surrogate renal outcomes as declining of estimated glomerular filtration rate (eGFR) > 30%, > 50%, and chronic kidney disease (CKD) with eGFR < 60 mL/min. They focused on the 1-year eGFR, using paired t-test, in patients treated with or without rt-PA. Favorable renal outcomes were observed among patients receiving rt-PA for acute ischemic stroke. In rt-PA patients with advanced CKD, they noted no increased incidence of intracerebral hemorrhage.
