False-positive results in a population-based colorectal screening program: Cumulative risk from 2000 to 2017 with biennial screening

Ibáñez-Sanz G, Garcia M, Milà N, et al. - Researchers undertook this retrospective cohort study to estimate the combined risk of a false-positive (FP) result in a fecal occult blood test (FOBT) through 7 screening rounds, as well as to determine its related factors in a population-based colorectal cancer screening program. The cumulative FP risk was estimated to be 16.3%, adjusted by age, gender, and type of test, from 2000 to 2017. The median number of screens was 2. The likelihood of an FP with 4 screening rounds of guaiac-based test and immunochemical test was 7.3% and 12.4%, respectively, in participants who started screening at age 50 years. Among screenees, the factors that were related to an FP result included age, the fecal immunochemical test, first screening, and the number of personal screens. In this study, the cumulative risk of an FP in colorectal screening using FOBT appeared acceptable as the colonoscopy, with its high accuracy, lengthens the time until additional colorectal screening is needed, while complication rates are low.
