Factors that influence survival in high-grade serous ovarian cancer: A complex relationship between molecular subtype, disease dissemination, and operability

Torres D, et al. - Researchers examined patients undergoing primary surgery for stage III-IV high-grade serous ovarian cancer (HGSOC) at Mayo Clinic from 1994 to 2011, to determine the association between molecular subtype, intraperitoneal (IP) disease dissemination patterns, resectability, and overall survival (OS). Study participants were grouped into three IP disease dissemination patterns: upper abdominal or miliary; lower abdominal; and pelvic. Based on the findings, a paradigm in which molecular subtype is an important driver of dissemination pattern was supported; the impact of this on resectability and survival was evident. Therefore, the association of mesenchymal subtype with much lower rates of complete resection was shown, though RD0 continued to be identified as the most important independent predictor of survival.
