Factors influencing the outcome of goniotomy and trabeculotomy in primary congenital glaucoma

El Sayed Y, et al. - In patients aged ≤12 years who had goniotomy or trabeculotomy for primary congenital glaucoma, researchers analyzed the prognostic factors that had an impact on intraocular pressure (IOP) reduction and success rates. Goniotomy was performed on 120 eyes and trabeculotomy was performed on 332 eyes. The median IOP reduction was 19.4% and 36.8% in the goniotomy and trabeculotomy groups, respectively. A positive correlation of median IOP reduction with a high initial IOP was seen in the goniotomy group, while it was mostly impacted by preoperative IOP, corneal clarity, gender and consanguinity in the trabeculotomy group. The preoperative cup-to-disc ratio affected failure rate in the goniotomy group, while it showed a robust correlation to positive consanguinity, higher preoperative IOP, female gender and younger age at surgery in the trabeculotomy group. Overall, trabeculotomy appeared more effective than goniotomy in primary congenital glaucoma.
