Factors for modifying the termination of resuscitation rule in out-of-hospital cardiac arrest

Yoon JC, et al. - In this prospective, multi-center, registry-based study, researchers validated the current basic life support (BLS) termination of resuscitation (TOR) rule in out-of-hospital cardiac arrest (OHCA) with using nationwide Korean Cardiac Arrest Research Consortium (KoCARC) registry, as well as, investigated the factors for modifying the rules. For this purpose, they used the non-traumatic OHCA registry data. They identified independent factors related to poor neurologic outcome, to propose new KoCARC TOR rules. High false positive rate (5.9%) for predicting poor neurologic outcome was shown by the current BLS TOR rule in this recent nationwide cohort. It was expected that unwarranted mortality could be attenuated with the use of new KoCARC TOR rules, application of two new factors (asystole as initial rhythm and age> 60 years) with BLS TOR rule.
