Factors associated with the degree of glycemic deterioration among patients with type 2 diabetes who dropped out of diabetes care: A longitudinal analysis using medical claims and health checkup data in Japan

Ihana-Sugiyama N, Sugiyama T, Imai K, et al. - Among adult Japanese workers with type 2 diabetes who discontinued diabetes care, worsening of glycemic control was evident mainly in male employees during treatment interruptions. However, great variations in HbA1c changes were evident based on baseline HbA1c and antidiabetic medication type.
This is a retrospective analysis of 1,125 adult Japanese workers with type 2 diabetes (mean age: 51.2 years) whose follow-up HbA1c rose to 7.6% after a mean 9.3-month dropout from diabetes care.
HbA1c deterioration was related to higher baseline HbA1c and sulfonylurea or insulin use.
Small alterations in HbA1c were evident in patients without antidiabetic medication, but about 2% or more rise in HbA1c was observed in those using sulfonylureas or insulin even when maintaining reasonable glycemic control before dropping out.
It is crucial to care for patients at risk of worsening glycemic control because of treatment dropout, particularly those using sulfonylurea and insulin.
