Extreme risk category: High prevalence among stable coronary patients and an emerging widening treatment gap in achieving LDL-cholesterol less than 55 mg/dL

Rallidis LS, et al. - Researchers sought to identify the percentage of patients with stable coronary artery disease (CAD) who are at extreme cardiovascular (CV) risk. In addition, they explored how achievable the new low-density lipoprotein cholesterol (LDL-C) goal of <55 mg/dL (1.4 mmol/L) for patients at “extreme risk” is. On analyzing 1,629 consecutive patients ≤ 80 years with stable CAD, they noted that more than half of these patients were at extreme CV risk. Very few (~5%) of these could achieve LDL-C levels < 55 mg/dL. To bridge the treatment gap, maximally-tolerated high-intensity statin combined with ezetimibe, if necessary, was recommended. Selected cases might require the addition of PCSK9 inhibitors.
