Extent of lateral neck dissection for papillary thyroid microcarcinomas

Lee JH, et al. - In this study, investigators examined if the size of primary tumor should be considered while deciding lateral neck dissection extent for papillary thyroid microcarcinomas (PTMCs). From 2009 to 2015, they compared subjects with PTMC and papillary thyroid carcinoma larger than 1 cm who received lateral lymph node (LN) dissection. Frequent metachronous metastasis, multifocality, less involvement of lower third thyroid, lower metastatic central LN counts and ratio, and lower metastatic total LN ratio were noted in PTMC group. No difference in lateral LN metastatic pattern was observed in the two groups. They noticed that men and lymphovascular invasion were more commonly found in PTMCs with synchronous lateral LN metastatic events in comparison to metachronous presentations.
