Exploring the link between tumour metabolism and succinate dehydrogenase deficiency: A 18F-FDOPA PET/CT study in head and neck paragangliomas

Reichert T, Fahkry N, Lavieille JP, et al. - Given that phaeochromocytoma and paraganglioma (PPGL) related to succinate dehydrogenase subunits mutations (SDHx mutations) were less differentiated than other subgroups and therefore can lack to concentrate 18F-FDOPA, a precursor of catecholamines biosynthesis, however, this 18F-FDOPA negative phenotype was reported mostly in SDHx-PPGL of sympathetic origin, indicating that both genotype status and location could influence 18F-FDOPA uptake, researchers explored whether SDHx drives 18F-FDOPA uptake in presence of normal epinephrine/norepinephrine concentrations. The study sample consisted of 86 head and neck PPGL patients with normal metanephrines who had 18F-FDOPA PET/CT. According to this retrospective study, SDH deficiency or metastatic behavior in HNPGL is not likely to influence 18F-FDOPA uptake owing to their very-well differentiated status, even at a metastatic stage. It would be necessary to further explore the potential prognostic value of 18F-FDOPA uptake in the setting of metastatic PPGL of sympathetic origin.
