Evolution of length of stay after surgical and transcatheter aortic valve implantation over 8 years in 1849 patients >75 years of age and comparison between transfemoral and transsubclavian transcatheter aortic valve implantation

Anselmi A, et al. - In this analysis of 1,849 elderly (aged > 75) patients receiving surgical aortic valve implantation (SAVR) or transcatheter aortic valve implantation (TAVI) from 2009 to 2016, researchers focused on evolution of length of hospitalization (LoS) after SAVR or TAVI and its predictors. They found that, although all-cause mortality at 30 days was comparable for both strategies, TAVI provided a faster improvement in terms of LoS vs SAVR, over 8 years of activity. Transfemoral TAVI (TF) and transsubclavian TAVI (TS) had comparable outcomes in terms of LoS, rates of procedural success and complications. Even so, TF cohort had more frequent vascular access-related complications, suggesting the TS route is a valid alternative to the TF approach.
