Evolution of anosognosia in Alzheimer disease and its relationship to amyloid

Hanseeuw BJ, Scott MR, Sikkes SAM, et al. - Considering that a challenging manifestation of Alzheimer disease is unawareness, or anosognosia, of memory deficits, that has adverse influences on a patient's safety and decision-making, researchers analyzed alteration in awareness of memory abilities as well as its link with beta-amyloid (Aß) burden in a large cohort (N = 1,070) of people across the disease spectrum. Utilizing the disparity between mean participant and partner report on the Everyday Cognition scale, memory awareness was longitudinally evaluated (average number of visits = 4.3) and operationalized. Using [18F]florbetapir positron emission tomographic imaging, Aβ deposition was measured at baseline. According to results, Aβ anticipated longitudinal changes in memory awareness, so that awareness in participants with increased Aβ burden decreased more rapidly. Findings suggested an association of Aβ burden with a progressive decrease in self-awareness of memory deficits, reaching anosognosia nearly 3 years prior to dementia diagnosis.
