Evaluation of the Arabin cervical pessary for prevention of preterm birth in women with a twin pregnancy and short cervix (STOPPIT-2): An open-label randomized trial and updated meta-analysis

Norman JE, Norrie J, MacLennan G, et al. - In twin pregnancy, a common cause of perinatal mortality and morbidity include preterm-labour-associated preterm birth. Researchers aimed at testing their hypothesis that the Arabin pessary would aid in lowering preterm-labor-associated preterm birth by 40% or greater in women with a twin pregnancy and a short cervix. In this open-label randomized controlled trial conducted in 57 hospital antenatal clinics in the UK and Europe, 503 women with a cervical length of 35 mm or less were randomized to either standard care (according to National Institute for Health and Care Excellence [NICE] guidelines) or standard care and an Arabin pessary, inserted before 21 weeks gestation. Spontaneous onset of labor and birth before 34 weeks 0 days of gestation was assessed at the primary obstetric outcome; a composite adverse outcome defined the primary neonatal outcome. Per outcomes, the groups did not differ in rates of the primary obstetric and neonatal outcomes. They suggest that any decrease in preterm-labor-associated preterm birth because of the Arabin pessary is less than 40%.
