Evaluation of optimum diabetes care on glycemic control of patients with gestational diabetes during Ramadan fasting

Hassanein M, Abuelkheir S, Alsayyah F, et al. - Because there is limited evidence that assesses the effect of fasting in pregnant women during Ramadan, researchers sought to explore the safety of fasting in Ramadan in gestational diabetes mellitus (GDM), while understating the glycemic variability. Twenty-five GDM patients who wanted to fast were recruited and given optimal treatment including Ramadan-focused education and FreeStyle Libre Flash Continuous Glucose Monitoring (FSL-CGM) was used for 2-4 weeks of non-Ramadan days evaluation period plus 2-3 weeks during Ramadan and adjustment of medication. The study emphasizes the value of structured education to provide adequate treatment for diabetes management prior to Ramadan. Strikingly, FSL-CGM showed a substantial rise in hypoglycemia during Ramadan Fasting. There was an efficient reflection, immediately after Iftar, of hyperglycemic spikes. The findings were corroborated by earlier reports of a higher prevalence of hypoglycemia during Ramadan fasting in high-risk patients with diabetes under similar standards of treatment.
