Evaluation of older age and frailty as factors associated with depression and postoperative decision regret in patients undergoing major head and neck surgery

Thomas CM, Sklar MC, Su J, et al. - In patients undergoing major head and neck surgery, researchers ascertained if age and frailty were correlated with preoperative and/or worsening postoperative depression and postoperative decision regret and identified additional factors related to depression and decision regret. According to findings, the prevalence of preoperative moderate to severe depression was 9.6% and the prevalence of moderate to severe decision regret was 26.7% in this prospective cohort study involving 274 patients (mean [SD] age, 67.8 [9.5] years). In the prevalence of moderate to severe depression or decision regret, there was no difference based on age. A higher preoperative frailty score was not associated with decision regret but with depression. On multivariate analysis, the only factor linked to moderate to severe decision regret was preoperative depression. Knowing the prevalence and factors related to moderate to severe depression and decision regret can help to identify patients who would benefit from more comprehensive pre-operative therapy and multispecialty preoperative and postoperative assessment and care.
