Epinephrine for out of hospital cardiac arrest: A systematic review and meta-analysis of randomized controlled trials

Vargas M, et al. - Researchers sought to determine how epinephrine vs control treatments influence survival at admission, ROSC, survival to discharge, and a favorable neurologic outcome in adult patients during out-of-hospital cardiac arrest (OHCA). Searching MEDLINE and PubMed from inception to August 2018, they included 15 randomized controlled trials representing 20,716 OHCA adult patients. Relative to providing a placebo/no drugs, providing epinephrine led to an increase in survival to hospital discharge, ROSC and survival to hospital admission; however, no significant difference in favorable neurologic outcome was reported. They recommend using standard or high doses of epinephrine in OHCA, as they improved survival to hospital discharge.
