Epidemiological state, predictive model for mortality, and optimal management strategy for pancreatic injury: A multicentre nationwide cohort study

Shibahashi K, et al. - The epidemiology of pancreatic injury with identification of mortality predictors, and determination of the optimal management strategy were described. From the Japan Trauma Data Bank, researchers identified data of 743 (0.25%) patients with pancreatic injury, which were recorded between 2004 and 2017. Prognostic factors of mortality after pancreatic injury were the American Association for the Surgery of Trauma grade, Revised Trauma Scale score on arrival, age, and coexistence of severe abdominal injury aside from pancreatic injury. As per confounder-adjusted analysis, operative management was not identified as superior to non-operative management for survival. Findings suggest the non-operative management as possibly a reasonable strategy for select pancreatic injury patients, especially in institutions where expertise in interventional endoscopy is available.
