Epidemiological and clinical characteristics and outcome of monoclonal gammopathy of renal significance‐related lesions in Latin America
Nephrology May 08, 2020
Peña C, Schutz NP, Riva E, et al. - Given monoclonal gammopathy of renal significance (MGRS)‐associated lesions represent infrequent entities, and there is a lack of publications on these disorders in Latin America (LA), so, researchers investigated epidemiological as well as clinical features of these patients in LA, via this multicentre retrospective study. They analyzed patients who received a diagnosis of MGRS between 2012 and 2018, including overall 27 patients from Chile, Argentina, Ecuador and Uruguay. In 33% and in 26%, proliferative glomerulonephritis with monoclonal immunoglobulin deposits and amyloidosis were detected, respectively, and monoclonal immunoglobulin deposition disease was identified also in 26%. An anti‐plasma cell regimen was used to treat 20 patients, and a rituximab‐based regimen was implemented in 3 patients. The achievement of renal response in 56% was documented. Overall, findings revealed that the frequency of patients needing dialysis was higher than expected. PGNMD was revealed as the most common MGRS‐related lesion. Better response was observed in relation to early treatment. In LA, the mandatory steps to improve outcomes of this rare disease are: increasing awareness and promoting early diagnosis.
