Enlarged mediastinal lymph nodes in computed tomography are a valuable prognostic factor in non-small cell lung cancer patients with pathologically negative lymph nodes

Zheng Y, Huang Y, Bi G, et al. - Researchers conducted a retrospective cohort study involving 758 N0 non-small cell lung cancer patients who had a thin layer computer tomography (CT) scan to ascertain whether there is a correlation between the size of mediastinal lymph nodes (LN) and prognosis in pathological N0 patients. In this study, mediastinal LN enlargement was observed in 150 N0 patients (19.8%). Age, pathology and tumor recurrence were significantly associated with LN enlargement. Multivariate Cox analyses revealed that patients with a larger volume had a higher risk of tumor-associated death vs the normal group. Findings revealed a worse 5-year cancer-specific survival and a higher risk of recurrence among N0 patients with LN enlargement. The most useful prognostic factor in N0 patients was determined to be volume of LN.
