Enhancing problem list documentation in electronic health records using two methods: The example of prior splenectomy

McEvoy D, et al. – Quality improvement professionals often choose between patient–specific interventions, like clinical decision support (CDS), and population–based interventions, like registries or care management. Researchers here determined the synergy of these two strategies, targeting the problem of procedure documentation for patients with a history of splenectomy. Findings suggested that population health documentation (PHD) and CDS strategies could best be used in conjunction to create a symbiotic relationship in which current problem and procedure documentation gaps were closed using PHD strategies, while new gaps were prevented through ongoing CDS interventions
