Endothelial cell density changes in the corneal center vs paracentral areas after descemet membrane endothelial keratoplasty

Musayeva A, Livny E, Dragnea DC, et al. - This retrospective cohort study was undertaken to determine if endothelial cell density (ECD) differs between central and paracentral areas after Descemet membrane endothelial keratoplasty (DMEK) and to identify the locations of the highest and lowest ECD. Central and paracentral ECDs of 30 eyes of 30 patients who had DMEK for Fuchs endothelial corneal dystrophy have been assessed. The central ECD decreased by 32 (± 11)% at 12 months postoperatively when compared with the preoperative donor ECD. Postoperative ECD has not been found to be uniform across the graft. In contrast to the density distribution in the normal endothelium, paracentral area ECDs have been found to be consistently lower than in the central area. Such variations were most pronounced in the superonasal area and persisted 12 months postoperatively. Therefore, central ECD after DMEK may not constitute an accurate proxy for the cell density of the entire graft.
